I authorize the Owner and the representative named in the order to transfer to the competent state authority the information and documents necessary to prepare, file and support a trademark application: identification and contact data of the applicant and representative, address, organization details, image of the sign, list of goods and services, power of attorney, payment documents and correspondence on the application.

I understand that part of the information about the application and the registered mark may become publicly available in the state registry to the extent established by law. The transfer is made to perform my order and legal requirements. Consent remains in force until completion of the procedure and mandatory document retention; withdrawal before filing stops the transfer, and after filing cannot cancel processing already performed by the state authority.
